{"id":40259,"date":"2013-10-01T09:32:42","date_gmt":"2013-10-01T13:32:42","guid":{"rendered":"http:\/\/www.bahamasb2b.com\/news\/?p=40259"},"modified":"2013-10-01T09:32:42","modified_gmt":"2013-10-01T13:32:42","slug":"human-error-likely-in-pineapple-air-incident","status":"publish","type":"post","link":"https:\/\/www.bahamasb2b.com\/news\/2013\/10\/human-error-likely-in-pineapple-air-incident","title":{"rendered":"Human Error Likely In Pineapple Air Incident"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-40276\" title=\"pineapple-air\" src=\"https:\/\/www.bahamasb2b.com\/news\/wp-content\/uploads\/2013\/10\/pineapple-air.jpg\" alt=\"\" width=\"200\" height=\"150\" srcset=\"https:\/\/www.bahamasb2b.com\/news\/wp-content\/uploads\/2013\/10\/pineapple-air.jpg 200w, https:\/\/www.bahamasb2b.com\/news\/wp-content\/uploads\/2013\/10\/pineapple-air-150x112.jpg 150w\" sizes=\"auto, (max-width: 200px) 100vw, 200px\" \/>Human error may have led to the door of a Pineapple Air plane \u201cflying open\u201d mid-flight last week, according to Hubert Adderley, manager at the Flight Standards Inspectorate.<\/p>\n<p>Adderley said the Civil Aviation Department has nearly completed its investigation into the incident.<\/p>\n<p>He said equipment malfunction was ruled out. \u201cWhen we checked the system, the doors were working properly,\u201d Adderley told The Nassau Guardian.<\/p>\n<p>When asked if the situation happened due to human error, he said: \u201cThat\u2019s pretty much the conclusion we were able to draw from this.<\/p>\n<p>\u201cIt\u2019s still difficult to prove that someone left the door open or did not close it properly or that there was a malfunction. Everything worked the way it should.\u201d<\/p>\n<p>When asked if the department would penalize the airline, Adderley said no. \u201cWe don\u2019t penalize our people for an incident,\u201d he said.<\/p>\n<p>Adderley previously told The Guardian that after the investigation was completed the airline may be asked to put extra precautions in place.<\/p>\n<p>The plane left Lynden Pindling International Airport (LPIA) last Sunday afternoon en route to San Salvador but returned to the airport shortly afterward.<\/p>\n<p>A San Salvador resident who was on the flight, who wished to remain anonymous, said she was sitting two rows in front of the door, which reportedly opened.<\/p>\n<p>\u201cThe lady who was next to the luggage compartment door screamed and everybody looked back and that\u2019s when we realized that the door was actually open,\u201d she said.<\/p>\n<p>\u201cShe was exposed to the outside and the other two people who were seated next to her grabbed her.\u201d<\/p>\n<p>The passenger said the plane was flying over a lake at the time, which added to her fears.
